Existing Collegiate Chapter Re-charter & Audit Form

This form is being distributed to all chapters for the purpose of renewal and auditing. We want to be able to reach out to all chapters confidently, so our leaders and members can stay informed about the happenings of the Society. PLEASE NOTE that while we do need you to provide the names of your Chapter Executive Officers (CEOs), it is the responsibility of your chapter president (this applies to collegiate and professional chapters only) to delegate/appoint the chapter e-board positions. For NSBE Jr chapters, the chapter advisor is responsible for this process. The instructions can be obtained by emailing membership@nsbe.org if needed.
Before completing this form, please ensure that your chapter meets the following requirements:

Collegiate chapters are required to have 1 chapter advisor, 5 officers (President, Vice President, Secretary, Treasurer, Programs Chair, and Senator), and a total of 10 registered/active members. If these stipulations aren't met, your chapter will not be recognized as active on the national level. If you do not meet the active chapter stipulations, still complete the form and request an exception using the directions listed below. Your chapter can be activated at a later date.
