Required Registration Roster
To ensure your group registration is processed accurately, please follow these guidelines:
Submission Format:All registration rosters must be submitted in Excel format using the official template provided.
Do not submit PDFs or other file formats.
Information to Include: For each participant, ensure the following fields are completed:
- First Name
- Last Name
- Email Address (each email must be unique; no duplicates)
- Member Type (e.g., Collegiate, PCI, Professional)
- Emergency Contact Name and Phone Number
Additional Notes: Be sure to include all chaperones on the group registration Excel sheet. Once your form and Excel template have been submitted, you will receive an invoice confirming payment details.

If paying by check, mail your payment to the address below and include your Chapter Name and Invoice Number:
